WHAT TO EXPECT:
Project Commitment
Over the course of the 10-month project, participants will undertake the following activities:
- Team Engagement: Identify a clinician and staff member to participate in the project and monthly meetings
- Data Submission: Provide demographic and patient population data before the first meeting
- Strategy Development: Complete a SMARTIE worksheet outlining your literacy strategy
- Targeted Guidance: Meet one-on-one with UHF staff and/or program mentors to discuss project development and implementation
- Cohort Meetings: Participate in monthly virtual cohort meetings, focused on resources, best practices, integration strategy, and problem-solving approaches
- Reporting: Complete and submit a comprehensive report detailing the development, implementation, and impact of the project by December 2024
Benefits of Participating
Selected sites will:
- Receive a $7,500 stipend and a one-time book order from UHF to enhance your literary resources
- Learn from subject matter experts about evidence-based literacy models and the value of incorporating a literacy strategy into primary care
- Receive technical assistance and guidance on design and implementation
- Have access to community resources and opportunities to collaborate with community partners to amplify project impact
- Engage with other participating sites, sharing feedback, best practices, and challenges

Gabriela began her journey with UHF in 2015, providing administrative support to the Program and Policy department. In 2022, Gabriela transitioned to the Clinical Community Partnerships team, where she took on the role of program manager. In this capacity, she spearheaded initiatives aimed at advancing early childhood literacy and health equity. Presently, Gabriela brings her expertise to the Health Equity Fellowship team, continuing her role as a program manager. Additionally, she serves as a facilitator for UHF's Diversity and Inclusion Strategies working group and is a valued member of the Board Diversity, Equity, and Inclusion working group, championing diversity efforts within the organization.
Before joining UHF Gabriela was Executive and Project Assistant at Scholastic Media, where she provided administrative support to executives and assisted on special projects related to the company’s reading app.
Gabriela holds a Master’s degree in Liberal Studies from The Graduate Center, City University of New York, and a Bachelor of Arts from Long Island University.
