Innovative approaches to implementing outcomes-based management in primary care practice are presented in this webinar by Dr. Gene Nelson, Dartmouth Institute for Health Policy and Clinical Practice, who shows how patient-reported outcome measures (PROMs) can be combined with patient-centered outcome measures (PCOMs) and patient-reported experience measures (PREMS) to create an integrated understanding of what matters to a patient and where the patient falls within a "value compass" of functional outcomes, clinical outcomes, experience of care (versus need), and cost of care.
