What we're accomplishing with your donations.
The Primary Care Core Measures are a set of measures across six domains that aim to provide multipayer performance data for primary care practices in New York State's primary care transformation program. The majority of measures are National Quality Forum-endorsed, and aligned with the requirements of several national and state programs. The measure set was developed with input from the Integrated Care Workgroup, a State-appointed multi-stakeholder group of payers, purchasers, providers, and consumers, and continues to evolve as the program is implemented.
