The APC Core Measures are a set of 28 measures across six domains that aim to provide multipayer performance data for primary care practices in New York State's Advanced Primary Care (APC) program. The majority of measures are National Quality Forum-endorsed, and aligned with the requirements of several national and state programs. The measure set was developed with input from the Integrated Care Workgroup, a State-appointed multi-stakeholder group of payers, purchasers, providers, and consumers, and continues to evolve as APC is implemented.
A compendium of resources related to the 13 measures to be included in the first APC Scorecard is available here.
