The New York State Primary Care Core Measure Set comprises 27 vetted, standardized measures that are meant to reflect the quality of care that a primary care practice delivers to its panel of patients.  Most of the measures are National Quality Forum-endorsed, and aligned with the quality reporting requirements of several national and state programs.  The measure set was initially developed with input from the Integrated Care Workgroup, a State-appointed multi-stakeholder group of payers, purchasers, providers, and consumers.  The New York State Department of Health continues to engage multistakeholders in its efforts to align the measure set with advances in measurement and performance monitoring.
