NEW YORK, NEW YORK February 26, 2018—United Hospital Fund (UHF) has awarded two grants totaling $219,107 for projects aimed at improving primary care by addressing the social, economic, and environmental stresses that can negatively impact health The grants are part of UHF's mission to build a more effective health care system for every New Yorker.
A one-year grant of $119,107 was awarded to Community Healthcare Network (CHN), a nonprofit organization providing primary care, mental health care, and supportive social services to more than 58,000 people a year in underserved communities throughout New York City. CHN's project, Integrating Social Determinants of Health in Primary Care Practice, will develop and vet a process for creating effective partnerships between primary care practices and community-based organizations to address social determinants of health, such as housing insecurity. The project will also develop a screening, referral, and feedback process that encourages effective follow-up between primary care providers and community-based social service organizations.
A $100,000 one-year grant was awarded to Public Agenda, a nonprofit research organization based in New York, for the project Parent Perspectives on Pediatric Primary Care Innovations. The project will develop, conduct, and analyze eight focus groups in New York City, to understand low-income parents' openness and perspectives to primary-care based innovations, particularly those that address social determinants that influence child development.
The grants complement UHF's ongoing efforts to improve primary care and access to health care and supportive services for underserved New Yorkers.
Both projects will start immediately.
