Bridging Acute and Post-Acute Care in a Changing Environment
The panel will explore how value-based payment models and quality reporting are driving changes in the organization and delivery of acute and post-acute care (PAC) services. Speakers will address the case for bridging silos between different care settings and associated challenges, the emergence of PAC networks and their implications for improving quality across the care continuum and containing costs, and the difficulties consumers, patients, and families continue to face in making informed decisions about PAC until more of the promises of health reform are realized.
