New York State's CARE Act: A Guide for Patients and Family Caregivers

The New York State CARE (Caregiver Advise, Record, and Enable) Act requires hospitals to give patients the opportunity to name a family caregiver willing to be part of planning for discharge and providing care at home; New York is one of more than 30 states that have adopted similar legislation.
