Together on Diabetes-NYC/Isabella Geriatric Center

Since January 2011, the United Hospital Fund has been developing and testing a diabetes community control model focused on seniors. Supported by a $2.8 million, three-year grant from the Bristol-Myers Squibb Foundation, the Fund's Together on Diabetes-NYC program in Washington Heights has forged partnerships among seniors, community-based organizations, health care providers, and other community stakeholders to improve diabetes self-management in seniors, improve clinical control, and reduce diabetes-related hospital and emergency room utilization. This grant will enable Isabella Geriatric Center to continue in its role as program anchor through June 2014.
