Since January 2011, the United Hospital Fund has been developing and testing a model program in Washington Heights to help seniors better understand and manage their diabetes. More than 1,500 seniors are currently enrolled in Together on Diabetes-NYC. The grant to Isabella will support its continuing role in participant recruitment and program promotion. The grants to the other three participating community organizations will extend their capacity to provide diabetes-related services. The grant to City Harvest will help provide a diabetes-friendly cooking and nutrition program at the four community sites.
