For patients and family caregivers, the day of discharge from a hospital is often fraught with tension, confusion, and apprehension. To address this problem, in February 2013, the Fund awarded grants totaling $150,000 to launch the Day of Transition Initiative, through which several hospitals and certified home health agencies analyzed their current transition procedures and began to develop new protocols to improve communicating and care during this stressful period. With this new grant, Metropolitan Hospital Center will continue to build on its work, further modifying and evaluating its new approaches. Metropolitan and its home care partner, the New York City Health and Hospitals Corporation's Health and Home Care, will test ways to improve the exchange of patient and caregiver information, including the post-discharge medication regimen.
