What we're accomplishing with your donations.
The hospital will develop the Seniors Coaching Seniors volunteer program to provide peer coaching support to low-income, socially isolated seniors who are at moderate risk of poor health outcomes or readmission to the hospital. Five to ten volunteer coaches, ages 65 and over, will be recruited and trained to call patients over a four- to eight-week period to ensure they understand their medication regimens, follow post-discharge instructions, and keep medical appointments.
