Quality Collaborative, Fall/Winter 2016/17
The lead story in this issue looks back at the significant accomplishments achieved through the IMPACT (Improving Processes and Care Transitions) to Reduce Readmissions Collaborative over the past three years to improve transitions between hospitals and nursing homes, and hospitals and home health care organizations. Other stories look at the continuing progress achieved through the New York State Partnership for Patients in reducing hospital-acquired conditions, and the recent successes of the Antibiotic Stewardship Program. Lastly, interviews with two current fellows of the GNYHA/UHF Clinical Quality Fellowship Program provide looks at their “capstone projects” addressing outpatient and ambulatory care issues.
