Carol Levine: Family Caregivers and Case Managers Working Together to Coordinate Care

The article points out that case managers are often best situated to find out what patients and families actually understand, not just what they have been told, and can assess what is needed to make a care plan work. The article also offers suggestions to case managers on how to build family caregiver engagement into the care plan, using a new tool of the Project RED toolkit and two new guides on care coordination, all developed directly by or in collaboration with the United Hospital Fund.
