Isabella Geriatric Center/Together on Diabetes-NYC

Since 2011, the United Hospital Fund—with a three-year, $2.8 million grant from the Bristol-Myers Squibb Foundation—has been developing a model community partnership program in Washington Heights to help seniors manage their diabetes and reduce diabetes-related hospital use. Recent data indicate the program--Together on Diabetes-NYC--is having a positive impact; participants report they are feeling better, are doing more to manage their illness, and are more confident about their ability to handle their diabetes, and utilization and clinical data is expected in the coming months. Isabella Geriatric Center has been an important community anchor for the program, which offers services at Isabella and three other senior-serving community-based organizations. These services include diabetes education classes and coaching, exercise programs, foot check lectures and screenings, and cooking and nutrition courses by City Harvest. Isabella is committed to sustaining the program and will be entering into an agreement with a large health plan to reimburse for select Together on Diabetes-NYC services provided to the plan’s members beginning in the summer of 2015 (but retroactive to the date the contract with the plan is signed). Full implementation of this new reimbursement process will take time, and this grant will cover program expenses during this interim period.
