United Hospital Fund’s 2014 Medicaid Conference

The rollout of the Affordable Care Act, the upcoming introduction of billions of dollars in transformation incentives through the Delivery System Reform Incentive Payment program (DSRIP), and major reforms already underway in New York’s Medicaid program are bringing rapid and ongoing change to New York State’s health care environment. This United Hospital Fund conference will examine continuing State efforts to implement these advances and will focus attention on care management services for the most complex and costly beneficiaries, and the building of collaborative partnerships among and between health care providers and community-based organizations.

Jason Helgerson, Medicaid director and deputy commissioner of the New York State Department of Health, will offer a morning keynote address on the State’s ongoing Medicaid reforms and strategic priorities moving forward. Following his presentation, a panel of acute care, behavioral health, and community health leaders will discuss the lessons of collaborative partnership-building.

Keynoting the afternoon session, Bruce Chernof, president and chief executive officer of The SCAN Foundation, will address the critical common elements of care coordination and explore how to adapt them to the special needs of varying populations of Medicaid beneficiaries. An expert multidisciplinary panel will then explore how care coordination works in practice, and the issues and challenges associated with coordinating care for complex patients and their family caregivers.
